I’m not certain of the make but it looks very similar to the Craft Supplies Combination Chuck

or Sorby Precision combination chuck PCC2000

Spindle mounting thread is 25mm x 2mm CodeMC30

Probable fit for Apollo (except professional), some Sorby, Astra, Tyme Avon, Selbix Lathes

Everything in the photos is included.

In good used condition, clean and in good order.
