We have an exciting opportunity at our City Campus to join our Supported Learning Department as a Transition Officer on a permanent, full time (35 hours per week) basis.
The Transition Officer plays a key role in ensuring that the move from school to college for young people with an Education Health and Care Plan is smooth and successful.
Full participation to team working to drive an inclusive strategy whilst working independently on a day-to-day basis.
To establish strong networks and professional working relationships with a wide range of schools, colleges, parents, internal and external agencies across the Greater Manchester district to facilitate the effective transition of learners with a SEND into the college.
Working autonomously within multi-agency professional meetings. An ability to seek out additional information to enable informed and complex decisions using the available information for learners with a SEND.
