To manage and prioritise a designated caseloadTo tailor intervention to the individual patient's needs, circumstances and beliefsTo provide information and advice for those involved in patient/client care in order to encourage their active participation in care plans.To evaluate outcomes and provide written reports as requiredTo demonstrate clinical effectiveness by use of evidence based practice and outcome measures

Requirements:
