We have a superb opportunity available for a Social Care Practitioner to work in the Sensory Needs Service at Wokingham Borough Council.
As the Social Care Practitioner, you will work under the supervision of a registered practitioner to undertake direct work with individuals and their carers. The core functions of this post is to complete Assessments in line with the Care Act, agreeing and implementing care and support plans and the reviewing function.
You will play an integral role in supporting carers to continue in their caring role for as long as they are willing and able to do so.

Key Responsibilities:
To work as part of a team to provide information, advice and signposting to vulnerable adults and their carers. To receive referrals for Adult Social Care Services, prioritising and managing risk on a day-to-day basis. Managing a varied case load. Where necessary, arrange interim Adult Social Care Services, with an overarching role to ensure the safety and wellbeing of vulnerable residents and their carers. Initiate Self Directed Support, so a planning amount can be set for a Personal Budget Use a strengths based approach to provide advice and signposting to other sources of support as required. They will provide specialist services to people living within Wokingham Borough Council boundaries who may be deafblind, visually impaired, deaf, deafened, or hard of hearing through the provision of specialist assessment, rehabilitation, equipment, training and professional support and advice. Candidate Requirements:
