Locum Band 5 Community Nurse - Ad-Hoc

Ad Hoc Shifts based at two Community Hospitals in Dartford and Gravesend

09:00-17:00

Job Description:

We are seeking motivated and adaptable Locum Band 5 Community Nurses to join our healthcare team for ad hoc shifts. As a Community Nurse, you will provide essential nursing care and support to patients in their homes and within the community. Your role will involve a variety of clinical tasks and responsibilities to meet the diverse healthcare needs of our patients.

Key Responsibilities:

Patient Assessment and Care:

Conduct comprehensive assessments of patients in their homes to determine their healthcare needs. Administer prescribed medications and treatments, ensuring accurate documentation. Provide wound care, catheter care, and other clinical procedures as required. Monitor vital signs, observe for changes in condition, and promptly report any concerns to the appropriate healthcare professionals.

Health Promotion and Education:

Educate patients and their families on managing their health conditions and promoting wellness. Offer guidance on medication management, dietary needs, and lifestyle adjustments. Provide support for patients with chronic illnesses to help improve their quality of life.

Medication Management:

Prepare and administer medications, adhering to established protocols and safety guidelines. Ensure medication compliance and provide education on drug interactions and potential side effects.

Documentation and Reporting:

Maintain accurate and up-to-date electronic health records (EHR) for each patient encounter. Complete incident reports and communicate patient status changes to the nursing team and healthcare providers.

Collaboration and Communication:

Collaborate with multidisciplinary healthcare teams, including physicians, therapists, and social workers, to coordinate patient care. Communicate effectively with patients, families, and caregivers to address their concerns and questions.

Safe and Ethical Practice:

Adhere to professional standards, ethical guidelines, and infection control measures. Ensure the safety and well-being of patients during all care interactions.

Ad Hoc Shifts:

Be available for ad hoc shifts as needed, including weekends. Maintain flexibility in scheduling to accommodate patient needs and team requirements.

Qualifications and Requirements:

Registered Nurse (RN) qualification with valid nursing license. Community Nursing experience. Current Basic Life Support (BLS) certification. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities. Proficiency in electronic health record (EHR) systems. Compassionate and patient-centered approach to care. Ability to work independently and as part of a healthcare team. Reliable transportation and willingness to travel to patient homes within the community.
