As a Community Nurse, you will work as part of a dedicated MDT to provide comprehensive, holistic care to adults within their own homes, enabling local people to live as independently as possible with maximum quality of life. You will have a GP-aligned caseload and also work closely with other health and social care providers, including voluntary agencies and ambulance services, to facilitate effective care and minimise the time needed away from home.
This is a permanent Community Nurse (RN Adult) position; both full-time (37.5h) and part-time contracts can be considered. Flexible shift patterns can be offered.
Person specification:
(Essential) Registration with the NMC as a Registered Nurse (RN Adult).(Essential) Valid UK driving license and access to personal vehicle.(Essential) Ability to work under limited supervision. Previous experience as a Community Nurse is desirable but not essential preceptorship and new to community courses can be offered to student/NQ nurses and nurses moving out of inpatient settings.
