We are seeking a number of compassionate and dedicated Registered Learning Disability Nurse to join our Community Healthcare Teams across Bradford Districts, on permanent contracts, agenda for change,

The successful candidates will play a key role in providing specialised nursing care to individuals with learning disabilities within the community setting. The positions involve working closely with individuals, their families, and other healthcare professionals to deliver person-centered care and support.

Key Responsibilities:

Assessment and Care Planning:

Conduct comprehensive assessments of individuals with learning disabilities in the community to determine their physical, emotional, and social needs. Develop individualized care plans in collaboration with the individual, their families, and other members of the healthcare team.

Clinical Care and Treatment:

Administer and monitor medical treatments and interventions as prescribed. Provide ongoing support and monitoring for individuals with complex health needs, promoting their overall well-being.

Communication and Collaboration:

Communicate effectively with individuals, their families, and caregivers to understand their needs and preferences. Collaborate with community support services, social workers, psychologists, and other healthcare professionals to ensure coordinated and holistic care.

Advocacy:

Advocate for the rights and needs of individuals with learning disabilities in the community. Work to ensure that individuals have access to appropriate community resources and services.

Health Education and Promotion:

Provide health education to individuals and their families, empowering them to manage health conditions and promote overall well-being. Facilitate group sessions or workshops on topics relevant to learning disabilities and health promotion.

Crisis Intervention:

Respond promptly and effectively to crises or emergencies within the community, implementing appropriate interventions to ensure the safety and stability of individuals.

Documentation and Record Keeping:

Maintain accurate and up-to-date electronic health records, documenting assessments, care plans, and interventions according to regulatory standards. Ensure confidentiality and privacy of individuals' health information.

Community Integration:

Promote the inclusion and participation of individuals with learning disabilities within their local community. Collaborate with community organizations to create supportive and accessible environments.

Qualifications and Requirements:

Bachelor's degree in Nursing with a focus on Learning Disability Nursing. Current registration with the relevant nursing regulatory body. Experience in providing nursing care to individuals with learning disabilities in a community setting. Strong communication and interpersonal skills. Ability to work independently and collaboratively within a multidisciplinary team. Knowledge of relevant legislation and policies related to learning disability nursing in the community. Commitment to person-centered care and community integration.
