Job Title: Clinical Pharmacist

Job Description:

We are seeking an experienced and dedicated Clinical Pharmacist to join our esteemed healthcare team on a full-time basis. In this role, you will be responsible for conducting medication reviews and providing valuable expertise in a primary care setting.

Key Responsibilities:

1. Medication Reviews:

- Conduct comprehensive medication reviews for patients, ensuring the safe and effective use of prescribed medications.

- Collaborate with patients, healthcare professionals, and multidisciplinary teams to optimize medication management.

2. Clinical Expertise:

- Provide evidence-based guidance on medication-related queries to healthcare professionals and patients.

- Stay abreast of current pharmaceutical developments and best practices.

3 GP Surgery Experience:

- Demonstrate proficiency in GP surgery processes, including electronic prescribing systems and medication reconciliation.

- Work closely with GPs and other healthcare professionals to enhance overall patient care.

Qualifications and Requirements:

- Registered Pharmacist with valid GPhC registration.

- Proven experience as a Clinical Pharmacist in a GP surgery or primary care setting.

- Strong knowledge of medication management and therapeutic guidelines.

- Excellent communication and interpersonal skills.

- Ability to work independently and collaboratively within a healthcare team.
